The only accounts I've read is the idiot walked away very quickly and shady like. Draw your own conclusions. I say he ran away out of embarrassment. It's probably good that he did scurry away, because like I mentioned earlier, his rig blowing up in a crowd like that means he could find himself in quite the litigious pickle.
Obviously he wasn't. The mod most likely auto-fired in his pocket, or the switch got stuck, or the battery wrapper was compromised and shorted. Either way the mod experienced a continuous fire, and whatever coil he had built was drawing too much current from the battery.
This is why "pulse" firing is a farce.
He obviously wasn't using a safe chemistry IMR battery, because thermal runaway, fire, and explosion ensued.... This type of explosion happens pretty much exclusively with ICR batteries. IMR's vent, get extremely hot, but really never go BOOM when they go into thermal runaway. However, ICR batteries in thermal runaway... hide your kids, hide your wife.

My money is betting he was using a very high mAh (> 6000mAh) ICR 26650, and not the 3000mAh-3500mAh IMR ones on the market that are okay for e-cig use. Couple that with his idiotic build, a malfunction or him simply pocketing it in his pants the wrong way causing a continuous firing, and there you have it.
Correct, if you know what you are doing. Which IMO, does not include the people claiming they know what they are doing when following "pulse" discharge ratings and vaping < .1Ω
