All you do is get a latex tube, like this one Shop Watts 1' x 3/8" Latex Latex Hose at Lowes.com, slide it over the top end (that's the part where the tip goes).
Items needed:
1. juice
2. carto
3. latex tube - cut to a size that suits you
4. empty juice bottle
Instructions:
1. fill tube with juice
2. blow juice through the top of the cartomizer and into the empty container
3. repeat until satisfied
The juice leftover in the empty container can then be poured back into the original juice bottle. Once you're all done, just rinse out the tube with water.
This 3/8-in size fits snugly over the regular sized boge carto's. So if you have a larger sized carto, I'm guessing you'll need a larger sized tube.
