Only if there's a lot of juice in the reservoir. When using attys, I have little to no juice in there. When using cartos, I never have juice in the reservoir.
That's a great question. I was contemplating that myself last night. I'm waiting for the Grands to go on sale now.

One of two things might happen. If you have an atty that is really wet, the juice could travel towards the drip tip, possibly get into your mouth. Depends on how long you hold it pointed down. If you have juice in the drip well, like you really overdid the squonking, then that would probably travel down the outside of the atty toward your mouth, or run out of the drip well.
If you quickly tilt and vape with just a little juice in the atty and none in the drip well, you just might be fine.

It's not going to be any more or less likely with the reo than with any mod that you drip on, the juice cannot flow out of the bottle because the feed tube is all the way at the bottom so the only way juice will get to your mouth is if there is too much in the atty already or if its in the catch cup.
Should be fine unless you keep your attys extra juicy. I just did it on the two I currently have loaded, one with a 1.5 ohm dually carto, and the other with a 1.5 ohm 306. No juice. Now I didn't keep it tilted longer than the few seconds needed to get a vape.
WHAT A STUPID QUESTION!!!! No not really I had the same concern but even when I got my very first Reo (reonewb) I laid back on my sofa and never had any juice leaking out and I even overfed my atty and still nothing.

The one on the Grand is a 306 drip tip/pipe stem and its on a Cisco LR 306 (I had to sand the bottom part of the drip tip to get airflow) and the one on my Woodvil is a 510 drip tip on a Cisco LR306 (I had to do a bit of messing around to get it to fit - I cut an old carto up and the drip tip that comes with the 306 - slid one into the other and then into the pipe drip tip).
As you can probably tell, I like Cisco LR306 atties!
