You can extract it at home or it can be done as an industrial process that takes it from tobacco and purifies it. A few places on the web sell juice with 'naturally' extracted nic but the vast majority of it comes as an industrial chemical (pharmaceutical grade). The same as is used in the patch and gums.

At not too high concentrations it is a poison and is used as an insecticide. If you get high concentrations of it it can be dangerous (deadly) and should only be used by people who know what they are doing. Even in lower concentrations be careful, wear cloves, and wash it off your skin.

Nicotine can be purchased in many strengths. Safety precautions should be followed when handling straight nicotine Base, even in diluted form. Common sense things like NOT leaving bottles open or storing where children can easily come in contact.
Wearing plastic gloves and working on stable surfaces with absorbent material near by in case of spills.
If Nicotine solution is spilled, wipe it up and dispose of the rag/paper towels/etc.
If Nicotine solution gets on you, step to a sink and Rinse off.

I work with 100mg, have had the occasional incident and even gotten it on my arm. No ill responses, but I do clean up any accidental spillage immediately.
