I've tried looking around but cant find anywhere that this seems to been an issue for anyone else unfortunately.
Def looks messed up. It did that as soon as you tried to open the fill port? I dont know that tank either but I wouldn't think it's supposed to look like that. Really dont know what to tell ya.
Yea, first attempt to refill, my guess is that it got hinged in the wrong way when I tried to unscrew it.
Ive contacted Innokin support and hopefully they've will have a solution.
I fixed one like that by just giving it a solid smack on the top. It's just off by half a thread. Nothing to lose, right?
To protect the threads, I'd screw it onto a mod, then give it a good Rap or to on a solid surface. GL!

Anyhow, back to the picture...I'm confused because I thought that was a screw-off top fill. But in your picture it looks more like a side fill system where you pull up the top cap to reveal the side fill port.

It is a screw on cap, so the only thing I can say is to use a cloth on it to get a good grip. Remember righty tighty, lefty loosly. That means counter clockwise to loosen it back up.

You may have to take it to a vape shop, might be a simple fix.
Might wish to fill in your avatar location(city-state & country), it may help you in obtaining location specific information to your future question(s).
On the top right hand corner of this post you will find --"your name" next to "Inbox and Alerts"
Place the mouse pointer on your name,click. You will get a drop down and on the left hand side of this drop down you will find "Personal Details". Click Personal Details.
In Personal Details, in middle of screen in gray, you will find "Location" Could fill that box line in.
