Self-Feeding

I've been reading comments thinking about self-feeding. I think it would be useful if we could collectively come up with some trouble shooting guidelines for common issues. Looking through past posts, I've come up with the following causes for self-feeding.

Environmental Causes:
Heat
High Altitude
Change in Pressure (airplane)

Equipment Causes:
Clogged Atomizer
O-Ring not Properly Seated in Cap
Low Profile Adapter
Overly Thin juice
Some Atomizers Just Do It (?)
Over-filled Bottle (Thanks Buddafucco)

Human Causes:
Over Squonking
Setting mod Aside with Full Atomizer
Drawing Too Hard (thanks Rhaps)

Anyone think of anything else?

All size 510-801 needleless adapters with some juices. The height of 801 tube makes little difference as I've used all IKV sizes, standard, older and newer Mini, and Semi (no longer produced except for the ones I cut down) and those I've cut to different heights down from tall/standard. These have all been IKV and 1.8 to 2.5 ohm.
