a "pass through" does exactly what its called. passes the voltage though, if its a KR model then yes whatever the voltage is that your plugging it into, that is what will you get.
That's a gross over simplification, with nothing connected to it you are right. Once you attach an atty and press your button it's all about the current (amps) available and the resistance (ohms) of the atty.

With my cheap meter I get 3.2 ohms with a cartomizer. To get a true 5V requires about 1.6A (1600mA). If you were to plug the PT into a more common 5V @ 1000mA AC adapter you'd end up at 3.2V while actually vaping.

Also note the same amps (1.6A) and PT (using an apapter) with a Joye 510 atty (2.3 ohms) will only hit about 3.7V.
