How often should I be replacing cartomizers? Is there a tell-tale sign when they're ready to go? And do I absolutely have to clean them periodically or is that just to prolong cartomizer life?

How often to replace cartos depends on how much you vape and how well you take care of them. Cleaning them prolongs their life but you can just chuck them when they start to taste burnt or don't produce as much vapor as when they were new. Cartos are pretty cheap so lots of people just toss them and open a new one.
