Bavarian Cream. Only flavor I've ever dumped. Fortunately it was only a 3ml sample, so I only poured 2.9 ml down the drain. But hey, I have a spare 3ml bottle now.

I haven't tried too many different flavors, but I have a Chocolate/Orange/Coffee that is not good at all. The orange tastes watered down, the chocolate is non existent. So I added a little Atomic Fireball to it. Man, it really brought out the orange and made it totally delicious.

Very few pre-made flavors taste good to me.
The worst though has to be what is sold as Red Bull.
2 different vendors, both tasted exactly like Vicks44 cough syrup.

And Vicks44 is the epitome of nasty!

Cignot.com (one of my favorite suppliers, for the record) has a Chocolate Cream flavor, VG 26mg. I'm not exaggerating when I say it tastes like licking a dirty ashtray and then licking a 9v battery. Or at least what I think that would taste like.

To show how stupid I am, I tried it again later with a fresh atomizer just to be sure. <shiver>
