I'm going to jump in here with my own WAG, as the OP said she was getting enough nic with the lowest level patches and gum, so she may be sensitive to the nic levels. A lower level may help...:)

I smoked 1 1/2 PAD full flavor, and needed 36mg juice to beat the cravings, no headaches, no problems....BUT when I had tried patches I used the highest levels, and would still pull it off, smoke a cig and slap it back on. Gum I could chew through a 1/2 pack in no time, at the highest level.

If you're sensitive enough to the nicotine in the lowest levels of patches and gum, you sound to be the exact opposite of me. vaping at 6mg or 12mg may be the level you need, each person is different. There's no magic level to fit someone "because I used to smoke....", only a guesstimate starting level to adjust up or down depending on how you feel.

I agree with lowering the level. Which is my next step. But as I've stated I also mixed two brands today with one brand having an unverified ratio. As this brand in an 18mg also had me screaming with pain and broke my throat out (like strep) that's why I lean towards a pg sensitivity. I can't find any ratio on the website so will resort to calling them tom as they are local. I did drop my nicotine and plan on doing so again Friday and maybe try a different pg/vg ratio. I'd like to avoid moving the ratio if possible because I like the sweetness. Again yes I could quit using the lowest NRT in the past usually because the lozenges made my mouth ulcer and I couldn't handle that so just quit all together.

A higher VG level should increase the sweetness you like, and also would be less likely to be a cause of your issues....:)

p.s. I used to pop the lozenges like Tic-Tac...don't like to think what it did to my body...everyone's different...
