I added 4 ml of 48 mg to 30 ml of 12 mg. My math says that comes out to 18.6 mg.

You have 4ml*48mg/ml=192mg and 12mg/ml*30ml=360mg. So 360mg+192mg=552mg and divide by the total volume of 34ml to get 16.24mg/ml.
Your error was about 12%. Which isn't too bad since the nic levels of each are pretty low.
