Like Kent says, the main issue is on charge, because chargers can go faulty, or not recognize when the ecig control circuitry is faulty, and just keep pumping power into the battery. This is why you shouldn't leave these things on charge when asleep or out of the house.
One answer is to use a fireproof Li-Po charging sack to put it all in when on charge - then if it catches fire the incident should be controlled.
These
devices are safe in use because that's what the statistics say: none has ever exploded while in use, just while on charge. The dangers of single lithium cells are minuscule compared to those with two cells in series. Any
device with two cells in series needs substantial gas vents, even if it has electronic controls, because electronics are not reliable. At least, the type we use aren't: tiny circuit boards having 3 or even 4 amps pushed through them. This is not a recipe for reliability.
