I've been labeling my refills with a sharpie but have found that it rubs right off and then I can't tell what is what. A standard ball point pen doesn't seem to work at all. Any suggestions?

Get sticker labels to write on then put them on there? Probably not all that attractive but would get job done.
I use a black sharpie around the battery end. I only use them for 2-3 days, so yeah it wears off, but It's just legible enough for me to read. If not, then I get to play a game of "Guess that juice!"

I have DVD tops. Meaning the big round bin that covers your DVDs that you would create your own with.

Yup, I use labels I had that came with some old VHS tapes. Write on them and wrap them around my cartomizer. Works ok but make sure you don't wrap it around more then a little past were it needs to stick to itself. Too many layers made it hard to get the condoms on and off.

Extra-fine oil based paint pens will work well. Search on eBay (or google) for paint pens or paint markers and check that they're oil-based. I use Sharpie gold and silver for this. Fine-point might work but I find only extra-fine can get a few clear initials for the name and nic level.
The oil-based paint pens will not rub off with water, but they will blur with rubbing alcohol and will scrape off with a fingernail.

I did the Sharpie silver thing too but it wore off. I'll try the paint pens. I have a label maker but I don't like the look of a label wrapped around the cart like that.

the sharpie silver doesn't work for me but the pilot gold/silver paint pens do. comes off with a soaking in alcohol.
Sharpie also makes water-based silver markers (the kind you're likely to find in stores...I had to go online to find the oil-based paint markers/pens) so it can be a bit confusing.
i picked mine up at martin's--supermarket chain here that is owned by giant, which i think is bigger nationally; they have a staples (like the chain) mini-section. the pens look like this:
& they're fantastic, they show up on every color (black, whatever) although obv you need to use silver on the gold foil carts.
