Be careful with tightening, some of the clearos can be finicky. I've had my share of Hulk-out moments with them, too.

Holiday Inn Express guess: 99% chance blown/defective coil, 1% chance other.
(he said it pushed the center post back in after pulling it out, and the PV works with the new atty).
