I always get a free sample from MountBakerVapor as long as I spend $15 or more. Yes, if you use a coupon code and it brings your total before shipping below $15, your order will not qualify. You can also specify in the comments section what you want as your free sample. Be sure to include PG/VG ratios as well as nicotine strength, or they may just give you whatever.
I never get anything extra with my orders. :(

I never got samples either and couldn't figure out what everyone was doing to get them. Felt left out! But after I got a sample after my first order from Indigo, and then the second one, I figured out that they send a free 3ml sample with every order...my last order I got a peach tobacco they're beta testing (I feel special!).

I ordered juice from DFW Vapors for over a year before they finally sent me a koozie! I think I got a sample with that same shipment, too. Woo Hoo! 
I received 2 freebies of 15ml samples with my first order from ValueVapes but so far they are the only ones. Sept order is going to Fancee Juice and I'll get back on what happens

Almost every juice review I watch says they got a small sample juice with their order. I've ordered at least 4 10ml bottles of juice at a time from LJ's, BWB and Copper Creek so far. The Copper Creek order was bigger quantities too. 1 bottle of voo doo was 15ml and a 30ml bottle of house blend.
Even the Bulldog I just got in the mail today from OKC Vapes was missing the extra O-ring so I could line the air hole up better.
I feel so left out.
Try Nirvana Vapor. I ordered 3 30ml juice there and got to pick one 5ml sample juice.
If I am trying a new juice vendor, I always put in the order comments I am a new customer and look forward to trying there new juices, don't know if it helps, but had plenty of free samples

I live in OKC and have visited OKC Vapes store twice. First time for some tanks and lanyards, visit took 30 minutes and I was overcharged. The second time I ran out of juice and ran in for a 5ml bottle of Carmel and it took 45 minutes before I could get what I needed and get out! Way too crowded of a place and not enough employees, I've found other local stores with better juice that I can visit on my lunch break and still have time to eat
I have not ordered any juice online though, so no freebies for me. I like to try them before I buy!

I new to vaping, but after three days I knew that I was done with analog cigarettes. So I loaded up on cartomizers and bought a bunch of eliquid and juice. I ordered from dfwvapor and smokeless image. Both were decent size orders and neither came with any samples or extra goodies. I was a little sad too because I hear stories of everyone getting something special. Can anyone list vendors that they love that usually give some samples? Thanks!
The most extras I have ever received were from a B&M after spending $100 on kit and they didn't have a few things on my list, so the owner/operator threw in some Ekowool from her own private stash. I cannot even count how much money I have spent from several different places/sites and nothing extra, ever.. so don't feel alone. lol
MBV does a free 15 mL sample with a purchase of $15. If you don't request anything, they usually send out a random flavor, or a new one they're testing out. It's usually 12 mg 80/20. Last time I ordered I wanted to order Teaberry, but couldn't find it, so I requested it as a sample. They sent it, handwritten label and all, at my nic level, 18mL. I usually order 2 bottles at 15mL and one at 30mL and with 10% coupon comes to just over $15 so I can get my sample
