Tough to find in Canada...are you willing to order from the US? Phiniac.com makes some really cool pyrex tanks that can be customized to the hilt...but you pay for what you get

Mom and pop vapor shop sells short polycarbonate tanks with a fill port.
Health Cabin sells 4.5ml tanks for short cartos, i haven't seen them at Canadian or American vendors though.

I believe that Mopar over at Canvape has mentioned he will be getting them in. I couldn't give you an ETA on that though.
Cloudsofvapor is selling Siam Mods tanks, but they're also in the US. I could be wrong, but I'm pretty sure their tanks are 35 mm and the XLs are 45 mm just like cartomizers.

My regular size phiniac tank (in my avatar) takes the regular size cartos like boge and kanger. I asked about it first too, because I didn't want to have to fuss around with different length cartos to put in my tank.
Phiniac regular sized, Triple V from Valley Vapes, or Mini Sub tank from Mom & Pop Vapor Shop. I have all and they're equally awesome. The sub is the easiest to use, the Phiniac is the prettiest, and the Triple V is a beast.

Good stuff. What's the diff between the "need patience" method and the faster way from health cabin.
About $17 and a few weeks.
I haven't ordered them from them yet, but from everything I've read the regular registered mail can take up to a month or even a bit longer to get to parts of Canada (BC is the fastest of course) and EMS can get to people in Canada in a little as a few days to a week or so. The price is also weight based for the cheaper shipping, but usually only a few dollars, EMS is always $20 for anything I have in my cart.

No, but the process is simple enough- Cut the tube to desired length then punch the ends to match the width of the carto.
It came to me as i was looking at an NV smoke disposable tube, it has tough but flexible clear plastic tube with a thick rubber cap on both ends. cut, poke, poke done.
I have yet to actually do it though, but if you look at the picture you will see what I'm talking about.
Theses sell for around 9 bucks and are refillable

You can also buy two 10ml syringes for 30 cents each (or free if you're lucky at the drug store) and use the black plunger thingies for end caps and cut down one of the syringes for a tank tube. Voila!
