I didn't see it mentioned here, and I know it's mentioned on another thread, but I'll repeat it here, as it's what I do:
After I use any cart up the first time, premium or not, I pop the end off & grommet out and don't use them anymore.
I take one of the old silicone endcaps (the ones without any plugs attached) and use a ballpoint clicky pen, with the nub retracted, as a 'cookie cutter' (the tiny 'O' ring end of the tube where the pen nib comes out) to press a hole out of the end of the silicon condom. that is: I sit the condom on my laminated desk:
If I use a plastic pen (usually all I have) I sometimes need to mash/rub the whole thing against the desk a little to get it to cut through, but only takes a few seconds, and ends up with an 'over-cap' that works a charm.
As it slips on and off so easily (usually after you use it a little, some juice gets between and lubricates it a little) I tend to refill often, keeping it topped up mebbe 5-10 drops at a time.
This ensures I don't dry it out and keeps it tasting fresh for a good amount of time.
Yes, however, it is a little messier than the 'proper' refill and putting grommet and cap back on, but I'm fine with that.
Also, I like how I can keep it gripped in my teeth a lot easier with the silicone over the end.
