Form 8962 (from my 2015, when I paid premiums):

Untitled-2.jpg

Note "1095-A'.

My red blotches are for full-year coverage.

She mentioned "partial year", so hers will have the red blotches in the
corresponding months of coverage.

You're welcome.

(And they have until March 2nd to mail out those 1095's.)

!!!!!!!!!!!!!!!!!!!

Steve just got back from dropping stuff off at accountant.
I had no idea...there is a 1099 for the healthcare crap that we are forced to get ?
That counts as INCOME ???
Not income. Proof of coverage.

BASICALLY:
1095-A for private individuals who paid premiums (and other circumstances)
1095-B for private individuals who paid no premiums (Medicaid, NYS "Essential Plans" [or other states' equivalents]) (and other circumstances)
1095-C for employed individuals who have employers using ACA as insurance (and other circumstances)

infamous document that contains income information.

it very well could have been a 1099 hc which is also proof of coverage ;)

''1099-HC Forms
All 1099-HC forms to eligible subscribers will be post-marked by January 31, 2016. If you have not received a form by the first week of February 2016 ..... ''
1099-HC is Massachusettes only... I believe. At least it
STARTED as MA-only.

"... The main difference is one (Form 1095-x) is a federal form, while the other (Form 1099-HC
is a Massachusetts form. They both provide information about your heath coverage. This form
provides information about your health coverage, including who was covered, and when the coverage
was in effect."

Good morning, Snails!

Thanks everyone, for your good wishes! Left eye will be done on the 30th of March and the right eye a month later.

I put this off for over a year, but the last 6 months or so, vision really started to change. I stopped driving at night because every light started to look like a starburst.

I drove up to Jacksonville to Dr. Bowden's office to have it done. My regular eye doctor said he was the only one he trusted to do the surgery, so I was referred there. I would reccomend him to anyone for eye surgerys Gracie :D You're close to him up dere ain't cha ? :smokie:

Thanks Aggie, After a year putting this off, it gave me time to research! My neurologist highly recommended this ophthalmologist and uses him as his own! I also have quite a few friends who used him and they are very happy with the results.
I see that Dr. Bowden does have an excellent reputation.

I do not keep up with all states' tax codes.

Yeah. "1099" is so far away from "1095"... I cannot possibly imagine getting them mixed up.

In looking over the Form 1040 (and searching the PDF), there is no mention
of 1099-HC. It could be buried deep within the bowels of the instructions.
