Any electronic device can be made with defects or develop them. the big computer makers have battery or charger recalls every once in a while.

Three main issues here with our batteries and chargers.

The first and most likely is that you have a charger that over charges your batteries to a higher voltage above 4.2 and then stops charging. This is more likely to happen with cheaper chargers. So it's important to check fresh charged batteries coming off a new charger to make sure it's not charging them above 4.2 volts. Once verified as a good charger test a battery every month or two to make sure the charger is still good. Constant over charging can kill a battery in a few months.

The big fear but much less likely event is a charger that does not stop at either the correct voltage or even a higher voltage but keeps outputting a high charge current. This can trigger a thermal run away event in the battery, not good.

the third is a short develops and this can lead to a fire event, often described in recalls as a "Visually Verifiable Thermal Event"

for issues two and three it's important to always monitor your charger and keep them in a location away from combustibles so they can short, spark smoke, melt etc. until a circuit breaker trips without starting a fire. So sitting on that cool little wood stand, with the mail leaning against it on a wood desk, next to the curtains and plugged into a $2 extension cord is not the best idea.

I always have my charger plugged into a high quality surge suppressor, because they have a circuit breaker that will trip long before the house breaker does.

Full disclosure, I do not baby sit my charges, I do charge batteries over night, I do not charge when I'm not home.

I've got a Pila charger and what I've noticed is that when I take a battery out of it as soon as it's done charging the battery will feel warm. If I forget about the battery and leave it on for several hours or overnight when I go to take the charged battery out of the bay it feels cool. Whatever warms the batteries up during charge is absent after charging is complete. So the Pila chargers, to me at least, seem pretty safe if you accidentally forget your batteries on it.

Just looked around my house and I have 8 things that are designed to live in their charger when not in use.

5 cordless land line phones.
2 dustbuster vacuum cleaners.
1 Roomba robot vacuum cleaner.

But I am all for everyone doing whatever they feel is the safest for themselves and loved ones.

PS surprising to me was my count of over 20 rechargeable battery items in my house not counting vaping ..... I'd need a full time employee running around checking on them 20 hours a day deciding when to disconnect them all.

I bought one of these chargers over 2 years ago from SmokTek with a 3.7v 14500 box mod. It has a p/n on it, AG-126. I have used it with 14500, 18350, 18490, 18500, & 18650 batteries. I think at the time it cost me like $7.99 and it has outlasted 2 other chargers, (Trustfire and Xtar), I had purchased since. I use it daily, usually put batteries in prior to going to bed and pull them in the morning. I have never had a battery get hot or warm ever... It charges my batteries to 4.24v everytime... I was going to buy a Pila but as usual, thought they were overpriced and overhyped. Changed my mind after reading a while back of a couple people had failure problems With theirs. Only recently have I though about buying a new one, and if and when I do it will be a Nitecore. They seem to have the best ratings and are the most up to date technological wise. I like the fact they will charge Li-ion, IMR, Ni-MH, Ni-CD, or LiFePO4 batteries. I know the one I have won't last forever, but I'm hoping it does...

Glad that one checks out fine and is working for you. Mine like that would over charge and killed several batteries in about 4 months before I started double checking my chargers.
