To actually test an ecig battery you have to put it under load of the atty. That requires an adapter between the battery and the atomizer. Otherwise, you can't get to the connections. Unloaded voltage reading are meaningless.

Most of the adapters have been made by modifying a battery to atomizer adapter. They drill holes in the adapter and solder test points/wires internally on the adapter that you can hook to with the meter while the ecig is working.
