Kind of a broad question. The broad answer would be high temperatures. Maybe water or juice got onto the board and caused a short? Improper battery care? Could be any number of things. I don't think it is a common occurrence though.
Due you mean burn or get warm when used? I haven't heard of boards with a burning issue so I'm guessing what your talking about is heating up with use.
There are two main types of regulators, linear an switching.
Linear: Has been around longer and works fine but isn't as efficient. Less efficiency means the excess energy is heat which is felt through the regulator.
Switching: More efficient and widely used now, higher efficiency means less wasted energy as heat which is why you see "cool" mentioned in many of the names of the mods with these regulators and battery life is improved.
Both work fine and are still around, the madvapes box mod uses a linear regulator and is considered a great value by many. The ones with switching regulators usually carry a slightly higher price tag.

Corrosion can cause it to fry also..heat on the board will cause it to fail after awhile and they all get warm to some extent..but it's probably more likely something else like a switch would fail before the board. I haven't heard of any widespread failures in VV mods though and if under a 3% failure rate it would be considered a good device with electronics.

Most regulators will shutdown before they get hot enough to do serious damage, however the chips life shortens everytime its overheats, I don't understand why I have never seen anyone selling mods with a heatsink on the regulator to cut down on heat and thus lengthening the chips lifespan.
I don't see many posts of VV "mods" failing (at least not the tube "mods") so it must not be a big issue. I would bet the chips outlive the connectors or switches in the devices most of the time. You also would probably have to have vent holes for a heatsink to help and that would open the device to moisture or it would have to be on the side of the device.
My Lavatube does not even get warm, never mind hot. Also the electronics part is completely isolated from the atty so no juice can get into it.

That happens sometimes with electronics..if they are going to fail it's usually within a short period. Hard to say if the board burned in yours without taking it apart as it could be a wire or something else...
