Fossil fuel emissions have long been known to be killing people but phrasing it as you did is as good as saying "Malaria kills people not cancer", just because malaria kills more doesn't mean cancer isn't a problem.
That's yet another interesting response to this information (I put it up in many places).
It's the geographical and chronological research that is important. The chemical and toxic stuff is easy to say oh it's carcinogenic so it is the cause of all the cancers. I think smoking
tobacco certainly causes lung cancer but not in the numbers reported. Smoking was about before diesel (in large quantities). Smoking did not change but cancer did so I am prepared to believe that smoking did not cause the extra cancers. If the rise was diesel then it affected the smokers just as much as the non smokers so you should subtract that rise from the
tobacco causing cancers in smokers.
Your response was interesting as there seems to be resistance to accepting that we have been sold down the river on second hand smoke, all the bans and taxes and smokers made outcasts and so on. It is not surprising that the nonsmokers who were happy to get rid of the smokers were easy to persuade when smokers seem reluctant to accept they were duped.
