The FDA almost disapproved the nicotine inhaler in America years ago precisely because that fat white tube resembled a cigarette - and they don't want that. So our present e-cigs have a strike against them before agencies even consider their safety and efficacy. New models need to move away from the appearance of tobacco counterparts. A "medical" device appearance would be fine.
