Just curious. When mixing strengths to get the mg I want (especially when the bottle is full and hard to shake effectively), what goes to the top? PG? Flavoring? Nicotine? Or does the suspension hold reasonably well?

If you make a good recipe nothing will settle anywhere.
It should suspend, if not use a combo of PGA, VG, and PG, rather than just one exclusively. I have had to do this more and more. (I typically use about 1/3 to 1/2 of oil drops used in PGA... i.e. 2 drops clove oil, 1 drop of PGA, 10-20%VG, the rest PG, and PG based eliquid as the base, as of late.)
Leave a bit of air at the top (if not storing) because this helps mix when you shake the hell out of a bottle. I typically shake, flip, turn, shake, repeat. If it goes really cloudy and stays that way, try a few drops of PGA, or more PG.
(I can't really use VG based, as I get about NO Throat Hit, and tend to use VG constantly with no breaks. So I avoid VG >20%.)

Not only can you shake the bottle, put it between your hands kinda like you're praying and 'spin' the bottle between them vigorously (rubbing your hands to get warm type of motion). This helps to get a really good mix especially when the bottles are full.
I used to mix my strengths in the little cap before doing the cap refill but then I noticed that the higher nic stuff was going to the bottom of the cap and so I was only getting the lower nic stuff. Had to start mixing in a big bottle so I could shake it. Thinking nicotine might have a higher density so I shake everything now.
I put mine in a tube with a lanyard. A bag will do. This is the same method as condom cartomizer spin method. Put the bottle of juice in a take the lanyard, bag with shoe string, and spin it around. Works great!
