Interesting poll Kate. The only issue I see is that you may get some erroneous results as some people will check the things that they think others would think is important and not their true feelings.
For example- I know some people who say honesty is important yet they lie constantly.
Some may check the"Benevolent" box and then go around making fun of people.
Perhaps you are going for popular opinion as it relates to actions and behavior, in which case this poll should generate some idea of that anyway.
I do love the idea though. The psychological effect of posing these choices may help someone remember why we humans are here and awaken a desire to become just a little bit better in all these things every day.

Personally, I checked 1.Honest, 2.Wise, and 3.Independent. and all that goes with these three.
Not that I have reached perfection in any of these areas but it is what I strive for in my life.

difficult choices...choose your own ideals or base it on your perception of op's (other peoples) ideals. I based my answers on whats important to me...but that is not necessarily what I think makes a healthy human. I live by my own set of morals and ethics and most folks would think badly of me if they truly knew me. But I refuse to give up my basic beliefs of what makes me happy and I am not the type to say one thing and do another. Have been accused of being to honest for my own good. :D

Honesty...the truth will set you free...if you think you might have to lie about something...then don't do it.

Compassion...kindergarden rules apply

No fear or pushing past your fears...can't have fun or learn new things if your sitting on the sidelines and that includes being creative. Fear of failing dissappears the more practice you have at it...LOL
