Good note about the safety vents, had it been vented it appears that it would have had a good chance of containing the failure.
Now that my DX order for CR2's is finally in the 'Packaging' state, I see this thread! Glad you're OK parma. I have a 5v magnum mod I've been waiting to try until the batts get here. Now I'm not so sure....I guess I'd need to drill out some holes in the aluminum casing and the PVC both.
I've been using a FogLog mod w/ 2 Tenergy protected RCR123a's at 6v, maybe I'll finally order a regulator and some buttons and build a 5v nicostick w/ some protected 14500's now...russian roulette doesn't appeal to me very much.

I am pretty sure in this case it was a bad cell. But it is REALLY IMPORTANT for everyone to realize that a bad cell and explosion can happen with ANY battery of ANY brand. My only reason for posting this was just to let everyone know that it is NOT just one person who has suffered battery failure. y only point here was for everyone to realize there is danger and we should all exercise at least minimal caution.
I would think that a hole of about 1/16th inch right in the middle of the very bottom so as not to interfere with the spring would be fine. That is where I am adding the hole in all of my mods. I would rather they explode AWAY from my face instead of into it. Personally I don't ever want to deal with it at all, but even protected batteries can be faulty.
Another question...
I have a brand-new RCR123A just charged for the first time today. I got a few good puffs from it, then dead as a doornail. Is this a warning sign? After I verify that it is a dead battery, not a dead switch or something, I need to decide whether or not to pitch it. I'm scared to charge it. Yikes!
