A vaporiser (that's what they're called) doesn't have a temperature as high as the flashpoint of vegetable matter - so there's no smoke, but the nicotine vaporises just like it does in an e-cig.
Vaporizer - Wikipedia, the free encyclopedia

I bought one of these devices last Saturday, as my atomiser had died and it was the quickest way to get vaping again till my new atty arrives. It's not as convenient to use as an e-cig - the problem is that it quickly gets too hot for e-liquid, so that when you put a drop onto the heated "platform," it spits all over the room before you can get the glass cover screwed on. My workaround is to plug it in, wait exactly 2 minutes, and then load it with a drop or two of e-liquid, then quickly screw the lid on and use immediately. Definitely packs a punch that way, but the trouble is that it takes half an hour to cool back down again for a reload, which is boring. I might try adding a dimmer-switch so I can set the power level to give the right temperature all the time.

Another strange thing is that something condenses on the inside of the glass. If you keep it switched on after inhaling the initial vapour hit, the glass warms up, then this condensate vaporises and can be inhaled - it tastes as though it contains some nicotine, but it irritates the back of my throat like mad  (back on topic at last :D) - it doesn't make me cough, it only affect the back of my throat, and the soreness wears off as soon as I stop sucking on the tube. I've decided to stick to the "phase one" hit and waste the other stuff. But it's interesting that it's there at all - presumably something gets fractionated out of the e-liquid. There's absolutely no visible vapour in this secondary stuff. Maybe if I get this dimmer-switch set up, I'll be able to get the platform at the right temperature continuously, and then the glass ought to warm up enough to stop anything condensing on it.

The other thing I tried was to drop e-liquid onto a heated tablespoon and inhaling that.......it's a nice, smooth taste, quite sweet, and nothing like as tricky to do as I'd expected. Certainly didn't make me cough. But there is a risk of getting a badly burnt lip if you're not careful.

Neither of these nicotine delivery methods has given me any coughing or noticeable lung problems of any kind, and apart from the very temporary sore throat (which I avoid by not trying to vape the condensate), I haven't felt any negative health effects at all. It's a shame they look so illegal and are such a hassle to use, and I do get through e-liquid faster than before - quite a bit probably gets wasted. But anything's better than going back to smoking tobacco.

The sore throat and cough is probably caused by the burning of the fiber material in the cartridge. I find when I drip a spent cartridge, after awhile I get the itchy throat and non productive squeaky cough. Although many use polyester fiberfil in empty cartriges, then add the nicotine liquid, this may not be a healthy alternative to prefilled cartridges.
