Hello there and welcome to the nut house.........
First off how long have you been vaping for and what type of E-Cig and setup do you have?

The basic kit required for DIY mixing is indeed Nic juice, PG and or VG and flavors.
I personally buy Nicotine with 70% VG and 30% PG all in one bottle.
I and many others here will be glad to help you but would like a little more info first.

Make sure and do your homework. Things could go south quick if you don't especially with that nic level.
Here is a link to a good juice calculator. I just made some with it.
You will need these things.
Protective eye wear.
Disposable gloves.
Some sort of calculator.
PG., VG, Nicotine of a higher mg count than your desired nic level
Flavor.
Either an 5ml syringe or dropper and bigger if you plan on making big batches. Make a small one the first time you make a flavor.
A graduated cylinder starting at 10ml and one up. (not required but it helps).

Do your homework
Then is it is just a matter of doing it in a way you can be sure to follow the recipe each time.
Do not carry on a conversation or watch tv when you do that. LOL Long story. LOL
Clean up. Wash all your gear after you hands an glasses and wash you face and arms if you dont' wear a long sleeve shirt. Clean all your tools. If you make a mistake consider throwing that batch out. Better safe than sorry.
Here is the calculator I use.
