I'm sure they do.. when you try a swimsuit on for the first time you wear underwear. And besides, there is a protective sticker thing in the crotch area so it's all protected.
And people wash clothes after they buy them, so I doubt there's any difficulty with trying on swimwear and deciding against it and the store then selling to someone else.

Mens underwear does come wrapped in plastic.. and I'm not sure it's wrong if you take it out of the package and wear it..
But.. you know when you get a new RTA or RDA and if you don't wash it before using it you taste all that machine oil and its nasty? Same with mixing bowls or anything else out of a factory..
Well... your underwear or other clothing items come from factories just the same, running through various machines and being handled by many people before making it into that package...
So I wash everything... I actually thought everyone did... lol
