So. I'm sure I've nailed down the first twisting part. When it comes to the counter twisting part,
1) I find that I'm limited to making maybe 2 ft max at a time. Any longer, I get unequal counter twists (it's tighter nearer to the drill, and looser to my weight)
2) I still can't nail down how much I should be counter twisting. I happened to receive a 6 inch sample from someone who traded some Rene wires for some some MM (I think it was from seanap). From what I see, it's counter twisted such that all 3 strands are nicely and evenly wounded together, but the individual strand's folded over parts doesn't really open up. I've been counter twisting it slightly past that so that the individual wires opens up a tad. Not sure which is better though.
