I chain vape too. I can hit my PV every 30 seconds pretty easily and can go through 5-6 mil a day pretty easy. But I was a heavy smoker. 2 packs on a normal day and 3 packs on a heavy day.
I wouldn't worry about a nicotine OD. It's almost impossible to do without knowing you're doing it. The only real danger of nicotine is if you ingest a bunch all at once (suck as drinking e-liquid, etc). However, even that may not be enough -- there have been suicide attempts with e-liquid that failed.
The only concern I personally have is allergies. I am asthmatic and I notice sometimes if I chain vape, my chest feels "full" as if I can't breathe in all the way. I am certain something in the vapor is slightly irritating me, but I have no way of finding out what. It's nothing major and certainly not as bad as a full blown asthma attack, but still something that keeps me from vaping as much as I probably would.
Nothing in life is 100% safe, even medications the doctor prescribes. The way doctors look at "side-effects" is pretty simple: "Do the benefits of this treatment (along with possible side-effects) outweigh the harm of non-treatment?" If the answer is "yes" then treatment usually goes forward. In our case, I don't think many doctors can say that the benefits of vaping do not outweigh smoking analogs.
