In an effort to save myself some time I am going to try and condense it down to the basic concept and forgo the social back and forth. Basically what this mod is about. For specific instructions the link will take you to the first page which is typically where the awesome detailed instructions are posted.
There are like 40 pages and 30 pages and ect to read on these so this will just the be the super short all of them put together one. If I didn't understand the concept please let me know. I mean no offense if I do get it wrong.
In no particular order, well maybe a little bit of order. Will try to put like mods together if I can.

Buy putty here is the site for that mods featured putty Amazing Crafting Products
Shape it into a "plug", put a hole in it, let it harden and your set.
http://www.e-cigarette-forum.com/forum/cartridge-mods/53487-zfm-zero-filler-mod.html
Create a plug by cutting out the flat side of your cartridge cap. Cut a hole there are various sizes and shapes to try. Leave your plug as big as you can to prevent leaking from the sides in case of shrinking or deformation due to heat. I am personally going to try this out with tip from one of these since they already have predetermined "flow" rates I think I can dial into something that doesn't leak but does work. Probably wrong about that though.
Plastic Wedge Mod
This is three pages into another mod, but I like it
Cut a piece of plastic that has a hole/slit in it, it will fit down the cart diagonally, it will hold up your filler material. Suggested filler, "Web Vent Filters"
and in the same thread
Automatic Drip Mod (plastic edition)
Same kind of idea as above, but it's structure to hold the filler up is a bent thin piece of plastic.
http://www.e-cigarette-forum.com/forum/cartridge-mods/46916-reservoir-straw-plug-mod-rsp-mod.html
Cut a plastic straw, it will be used to hold up the filler. Suggested filler is "Scotch Brite blue non-scratch scrub pads (one without abrasives) or the blue fish filter foam."
The Blue Foam Dart Mod
Cut plastic straw, attach blue foam on one end
http://www.e-cigarette-forum.com/forum/cartridge-mods/40093-direct-drip-cart-mod-ddc.html
Take off the rubber grip of a pen, use it as the exterior structure for your new makeshift cart. Get a plastic funnel that will fit into the rubber grip. Suggest funnel is a "3ml disposable eye dropper". The funnel will direct your drops to it's target, and your set.

Not to cut the original creator out of any credit, but found the pictures and explanation on page 14 to be pretty good.
Page 14
Get round brass screens, shape into a cone, push into cart.
I don't really get how this doesn't leak, or if it doesn't leak how you refill it without taking the screens out. The only explanation I saw was not collaborated, but found a video of the liquid just going in. So all is well, plus tons of claims of it working helps.

Be sure to check the brass quite a bit for a possible unknown green build up. If you get that try another kind of brass screen, is the solution I read. Also can try other metal screens like stainless steel, and ect.

This one might too complicated to paraphrase accurately, but will try even though I do recommend reading this one. It's kind of beyond average tinkering, but fascinating.
Cut a small circle into the side of your cart, get a stainless steel spring and load it with a tiny stainless steel ball. The spring will hold the ball into the hole to create a seal. Make a silicone plug like from ZFMs post only make sure to use a hole and not X, or star cut. Push the ball, opens air flow, liquid goes into atomizer, release ball. If I ever feel super ambitious I will try to make this only instead of a ball use a very small steel column/button looking thing wielded into the ball so it doesn't spin so liquid doesn't get on the outside and on my finger. Maybe one day.
And those are all the cart mods I know of, and have an elementary level understanding of so far.
There are a quite a few mods where the mod is basically just changing the filler from the default material, and replace it with another material. With no structure added, or other changes I am reluctant to make an entry for each one.
Replacer material (besides the ones mentioned above) are
pyramid tea bag
blue aquarium foam
Cotton
You can google either and see pictures and info about the materials above.
