These things happen with all rechargeable batteries; laptops, cell phones, flashlights, RC toys. Just be safe and you'll be fine.

Being safe is important, yes. But being somewhat knowledgeable about li-ion technology and proper charging practices is just as important.
HMMM, I don't know if I understand what is being said here.
I have used a LOT of rechargeable batteries. I would like to know exactly what the issue is.
I use to be a little bit of a photographer. Spent several years very active on a Digital Photography web forum where everyone used lots of rechargeable batteries and never saw one thread about this kind of thing. My Fuji SLR camera used 1 CR123 and 4 AA batteries. They would take about 50 pictures before needing recharged so I carried about 3 sets. Then my flash attachment used another 4 AAs.
Then there was my family fun video cams. All the way from VHS to today's HD. Not to mention cell phones. My daughter has had her own cell phone since she was maybe 12 years old. (21 now)
WHAT ABOUT CHILDREN'S TOYS! They have them that the batteries are built in. With rechargeable batteries used in children's toys, I just don't think this issue has much to do with rechargeable batteries in general.
I've read on the internet that the quality of the battery is likely the real issue. Are these batteries that cheaply made?
If so I think I need to find the mod out there that can use a good battery and is still nearly the diameter of an EGO.

I also found this video about a man who was badly burned by an ecig battery explosion. It says that the guy had been using ecigs for two years. They have his name and face in the video. Does anyone know him?

i always charge my batteries overnight, plugged into a 500mah wall charger. have done it for years without a problem. now i'm not only reluctant to charge them overnight, i'm reluctant to use them at all. seriously, that could just of easily shot out of the other end of the tube and probably killed the guy.
is there a safer way of doing this? what about a box mod with brand name AA's inside it? any such thing exist? i know the chinese aren't too careful of their QA so i figure it's not if it happens but when it happens to me.

you're not wrong, i slung it after about 5 burns. ....ing horrible things aren't they? can't believe i stuck with them so long. so now i've got to find an alternative method of nicotine intake that doesn't involve a) chinese batteries or b) real cigs. might buy a hookah 
They are the worst.. I tried one for fun a couple weeks ago (after about 9 months without).. Terrible vapor production.. Flavor was the worst and the throat hit was pretty lame too.. lol

Questionable Quality Control in the Name of Rock Bottom Prices and Frantic Production Schedules to meet Demand.
Higher Voltage Batteries with Higher Power per Unit Density than Usually seen in Most Consumer Products.
Batteries that are Not Charged and Discharged Occasionally but Constantly.
Batteries Inside Extremely Confined Spaces which May or May Not have Adequate Venting.

All Batteries can Fail. Doesn’t matter what Type they are. It is what happens when it Fails that is Sometime more Relevant.
Say a Battery is in an Open or Well Vented Area and it Fails for Some Reason. A Plume of Smoke is Emitted and Perhaps it even catches on Fire. No Big Deal.
Now take that same Battery and Place it inside a Metal Sealed Tube. When it Fails this time there is Nowhere for the Gases to Go. This is the Concept of a Pipe Bomb. And if the Tube is 4 Inches from your face, well, you get the idea.
Do a Google search on exploding batteries and you'll see what I mean.

Just a little more safety to take here, people need to have many of sets of batteries, and let them rest before and after use. By this I mean charge your Batteries and then set them a side for a little rest before putting into your PV. Then once you use them do the same take them out and let them rest before you charge them. people who take there Batts right out of there PV slam them on the charger and charge them then put them right back into there PV is not the way to go that's constant use of the power cell and not good.
Resting the power cell is the best way, charging your Battery means your Battery is in use, just like running it in your Mods. Everyone using Mods should have 5-6 Set's of Batteries for that mod so they can comfortably rest the power cells in between usage. It's just a safety feature and one everyone should practice, you should also mark your set's as follows with a permanent sharpy. A-1, A-2 B-1, B-2 and so on, your top Batt will always drain 1st so switching them up top bottom, bottom on top next run and keep switching, this way one Batt doesn't always get depleted first and you end up running a Batt that's life of that power cell is halfway gone and the other is still fresh. just a few pointers for those stacking Batts. Same goes with using a single 18650 or single Batt, give them a rest in between charging state and using in your Mods.

How about a press release about this? Even if there's nothing that even manufacturers can do at present - especially when it's not even clear what equipment the man was using when it exploded, whether it was a mod - there's obviously a lot of concern from people in the vaping community. Press makes politics; often on partial information. I'd jump at the chance to sign a joint message about this horrible incident, extending good wishes to this man and his family; and I'm sure thousands of others would too.

Years ago, a fellow tech rep and I working working in a customer's office and heard a hissing sound. We looked back and it was coming from his tool bag. A 9v snap top battery (like you use in radios) had slid over and wedged against a tool and shorted. He grabbed it and threw it in the trash can, receiving a big blister on his hand in the process. A 9v uses 6 square 1.5v stacked cells inside.
Any battery can go into catastrophic melt down under the right circumstances.
Yeah, I think the point is that any battery can do this- and its rare, but pay attention. The chunk of aluminum I vape from has vent holes, but I hope I would notice if the sucker started heating up, and certainly would pay attention to any odd smell. It uses two 3.6v ultrafire batteries- any thoughts on the risk?
I am certainly much more cautious charging these than I am with any other electronic device...
